18 State intervention not only meant increased financial expenditure, it also contravened implicit assumptions about the role of government shared by both Whigs and Tories. Pressure from the new professional men, especially medics, for the state to take fuller responsibility for public health was severely qualified by dominant political and administrative ideologies. Government was to be kept cheap, and the central state was not to take on statutory powers for issues which were essentially the province of local authorities and voluntary initiatives.

Large-scale movements of organised revolt, along with more localised though no less significant cultural struggles, have had a major impact on sexual regulation and moral reform. Women’s struggles have been central here. In the field of sexuality they were in large part spearheaded by outrage against the medical establishment. From the repeal campaign against the Contagious Diseases Acts in the 1870s, through to the purity feminism of the suffrage movement, successive generations of women put together an alternative strategy—and alternative understandings of sex— which at moments routed the power of male experts.

This theory held that under certain unpredictable circumstances the atmosphere became charged with an epidemic influence, which turned malignant when combined with the effluvia of organic decomposition from the earth. The resulting miasma produced disease within the body. The choice of miasma theory was influenced both by the internal logic of medical discourse and by wider cultural and political considerations. Miasma fitted well with the scientific materialism of early social medicine. At the level of public debate it was graspable—effluvia could be pointed to and graphically illustrated.
