In the prior few years the transplantation of organs in guy has obtained exposure extraordinary in scientific background. the 1st center grafts have been coated by way of press, radio, and tv on a scal~ equiva lent to the scoop of the outbreak of a big struggle. Unwarranted and lavish optimism has been by way of sour feedback. This has undermined public self belief within the clinical occupation and heavily impeded development in a massive endeavour geared toward lowering human soreness. This unlucky scenario has arisen from common lack of expertise among the general public and the scientific career of the heritage, current achievements, and destiny power of organ grafting. brief statements via specialists, usually misquoted or reduce brief through tv interviewers, and misinformed derogatory pronouncements by way of prejudiced medically certified males, without wisdom of the sector, have produced a sorry kingdom of bewilderment. it's the goal of this publication to try to explain organ transplantation. the rules of organ transplantation are universal to all organs yet i'll confine lots of the dialogue to transplantation of 4 important organs, particularly the kidney, liver, middle, and lung. The techniques vary in detail but not in concept. Typing sera for histocompatibility antigens are not yet as clearly defined as are those for typing red cells. Histocompatibilityantibodies develop in the sera ofpatients who haverej ected tissue grafts, of patients who have had multiple blood transfusions, and of women who have had multiple pregnancies and have been immunized by their offspring. These sera are used to type lymphocytes ofpotential organ graft donors and recipients. If the lymphocytes of both donor and recipient are agglutinated or killed by a given typing serum, then as far as that serum is concerned thelymphocytes of both individuals have antigens in common.

Undoubtedly some of these cases are due to inadequate serological techniques, but another possibility is that serological typing gives no information on the immune potential of the recipient, which can be an important variable. Lymphocyte culture techniques may help in this assessment and they will now be discussed. LYMPHOCYTE CULTURE REACTIONS If blood lymphocytes from two individuals are cultured together in a nourishing medium they grow and divide. The proliferative activity can be assessed by measuring the incorporation of radioactive labelled materials by the cultured cells.

22 . A Rhesus monkey which had its own liver removed and received an orthotopic liver graft from another Rhesus monkey. The animal was given no immunosuppressive drugs and lived for seven months after operation. It died from an infection. PREVENTION OF REJECTION The discussion of specific antigen-induced protection of grafts from rejection or tolerance has been deliberately rather detailed, since this is our eventual aim in human organ transplantation. Less satisfactory immunosuppressive measures are available and will now be considered.
